Movement runs left to right through connected areas, with combat resolved as you go instead of through menus. The lead is a single fixed character, so the game is about surviving a route rather than building a party, and boss encounters close out each stretch of the map. The tone runs dark from the start: the material includes graphic violence, monsters and body horror, and nothing softens it.

The horror tone sits in the scenery as much as the writing: creatures, violence and monsters appear in the level design, and the animated art keeps scenes moving. Presentation stays two-dimensional throughout, so the look is closer to a drawn action game than to a 3D title. Grotesque enemies carry most of the atmosphere, and the Japanese release background shows in how they are designed.
